Responsibility
- Perform comprehensive physical assessments for patients in a home care setting, ensuring accurate and timely evaluations of their medical conditions.
- Conduct wound care and dressing changes according to best practices and provide IV cannulation if required
- Train and educate family caregivers on proper patient care techniques, including administering medication, mobility assistance, and basic nursing tasks.
- Ensure proper medication administration, educate patients and caregivers on the correct dosages, and monitor for side effects.
- Continuously monitor patients’ conditions, including vital signs, and report changes to the supervising healthcare professional.
- Assist patients with daily activities, such as bathing, dressing, grooming, and other personal care needs.
- Collaborate with multidisciplinary teams to develop and implement individualized care plans for patients.
- Provide emotional and psychological support to patients and their families, helping them cope with health conditions and treatment regimens.
- Maintain accurate and up-to-date patient records and ensure timely reporting of any significant changes in the patient’s condition.
Requirements
- Must be a full registered Staff Nurse with the Singapore Nursing Board (SNB)
